Post-dose Vancomycin peak draws
Hey folks, anything new out there in reference to drawing Vancomycin peaks 2 hrs. post dose. My understanding is trough management is still the norm and 1 hr post infusion peak is still the clinical acceptable norm. Feedback from others?? Mike Semon R.Ph. Caring Solutions Inc. Posted: 9/25/06
Response Summary
There were 5 Listserv responses to Mike Semon’s question. All responses clearly indicated that drawing Vancomycin peaks is rarely ordered. If ordered, the consensus indicates to draw levels one hour post infusion and in renal patients two hours post infusion. Trough level management is generally targeted at levels of 15-20 for the treatment of methicillin-resistant S. aureus pneumonia.

It appears that most clinicians have abandoned the routine practice of determining peak serum Vancomycin concentrations and most clinicians rely solely on monitoring trough serum concentrations for this antibiotic.
